This card's Japanese name is a reference to the Japanese name of the chapter of Yu-Gi-Oh! Duelist where "Great Moth" is first summoned, "Ultimate Perfect Form". Its English name likewise reflects the English name of the same chapter, "The Ultimate Great Moth".
This card is infamously difficult to reliably Summon due to the very specific conditions required, so much so that some players consider it bragging rights to be able to use this card at all. Several video games based on Yu-Gi-Oh! reward the player with a bonus if they manage to successfully Summon this card, because of that.
